It would be interesting to learn the view of fellow drivers.

Having driven some distance to the pick-up point, you have already used up none earning time and there is some vehicle and fuel usage to consider. At the pickup point the passenger approaches your vehicle and says that the other person(s) is on their way so won't be a moment. You discuss the destination, so everyone is happy.

Now that you are sitting at the pick-up point, know the destination and you could find yourself sitting for a further 5 or 10 minutes or even longer, at what point do you 'start' the trip?

To my opinion the meter should be running as soon as the pickup is confirmed and the passenger have a visual contact with you, confirming he's the passenger and asking you to wait. You are now booked and can not take other trips.
Alternatively you can wait and get paid for waiting time, the clock start ticking 2 minutes after your arrival to the pick up point. Surge rates apply (if applicable at time of pickup).

+1 . Either go for the waiting time > 2minutes or (cancel for no show after 5 minutes+ if he's taking forever and its surging all around)
Start the meter as soon as you have visual contact.
